Primary cardiac lymphoma (PCL) is a non-Hodgkin lymphoma involving the heart and/or pericardium, with the majority being intrapericardial tumors. The condition is uncommon, representing about 1% of primary cardiac tumors and 0.5% of extranodal lymphomas. The median age at diagnosis is 63 years. PCL often manifests as a cardiovascular event, such as shortness of breath, heart failure, cardiac rhythm disturbance, or pericardial effusion. PCL is highly aggressive and has a poor prognosis, with a median survival time of 12 months. We report a rare case of a 25-year-old man with PCL with clinical manifestations of atrial fibrillation and severe shortness of breath. He was diagnosed with PCL-type diffuse large B-cell lymphoma (DLBCL) through history, physical examination, echocardiography, radiological imaging, histopathology, and immunohistochemistry. PCL usually affects patients in their 60s but can also affect younger patients with cardiovascular manifestations and poor prognosis, as in our case.
