Diagnostic value of diffusion-weighted imaging-apparent diffusion coefficient (DWI-ADC) and dynamic contrast-enhanced (DCE) MRI 3-Tesla on histopathologic features of meningioma

Abstract

BACKGROUND: Meningiomas, primarily benign central nervous system tumors, have an incidence rate rising to 10.82 per 100,000 globally. Histologically confirmed meningiomas constitute 37.6% of primary central nervous system (CNS) tumors, with recurrence rates reaching 20% over 10 years. Utilizing diffusion-weighted imaging-apparent diffusion coefficient (DWI-ADC) and dynamic contrast-enhanced (DCE) Magnetic resonance imaging (MRI) has proven valuable for assessing meningioma grade, distinguishing tumor types, and guiding treatment. This study aims to evaluate the diagnostic efficacy of DWI-ADC and DCE MRI in histopathological grading of meningiomas in Makassar.METHODS: This observational analytic study, with a cross-sectional approach, was conducted from June to September 2024, using retrospective data from January 2021 to March 2024 on meningioma patients who underwent contrast-enhanced head MRI. Diagnoses were histopathologically confirmed. SPSS version 25.0 for Windows was used for all analyses, with univariate analysis describing variables. Bivariate analysis, including Mann-Whitney and χ2 tests, compared DWI-ADC and DCE MRI parameter values, with a significance level of P<0.05.RESULTS: A total of 50 samples were included in this study. ADC value analysis for distinguishing benign from atypical/malignant meningiomas showed 96% accuracy, with 95.3% sensitivity and 100% specificity. Max slope analysis achieved 76% accuracy, with 76.7% sensitivity and 71.4% specificity. DWI-ADC parameters had higher diagnostic accuracy than DCE in differentiating between benign and atypical/malignant meningiomas.CONCLUSIONS: ADC value and Max slope effectively differentiate benign from atypical/malignant meningiomas, with DWI-ADC parameters showing greater diagnostic accuracy than DCE for this distinction.
