It affects over 13.7 million people annually, with ischemic strokes comprising 85% of cases. Magnetic resonance imaging (MRI), particularly diffusion-weighted imaging (DWI), is crucial for early detection. This study explores the correlation between lactate levels and infarct volume in acute ischemic stroke using 3T MRI, aiming to improve diagnosis and outcomes.METHODS: This cross-sectional study analyzed the correlation between lactate spectroscopy values and DWI infarct volume in acute ischemic stroke patients using 3T MRI and modified Rankin Scale (mRS) scores. This study conducted the data from May 2023 until February 2024. Statistical analysis identified significant associations using SPSS v26, with Spearman methods applied based on data distribution, and significance defined as P<0.05.RESULTS: A total of 23 samples were included in this study with 65.2% were male, with most aged 56-65 years (39.1%). mRS Scores indicated 56.5% scored 4, showing moderate disability. Significant positive correlations were found between mRS Scores, lactate spectroscopy values, and DWI infarct volumes, with lactate levels rising with infarct volume. The lactate spectroscopy cut-off (77.872) achieved 94.1% sensitivity and 66.7% specificity, demonstrating strong prognostic potential.CONCLUSIONS: This study found strong correlations between DWI infarct volume, lactate levels, and mRS Scores, highlighting lactate spectroscopy as a valuable prognostic tool in acute ischemic stroke management. ## Keywords - Medicine - Modified Rankin Scale - Internal medicine - In vivo magnetic resonance spectroscopy - Correlation - Stroke (engine) - Magnetic resonance imaging - Cardiology - Diffusion MRI - Radiology - Infarction - Acute stroke - Nuclear medicine - Brain ischemia - Ischemic stroke - Cerebral infarction - Clinical significance - Cerebral blood flow - Biomarker - Predictive value of tests - Predictive value - Prospective cohort study --- Sumber: Discover Unhas — RIMS Universitas Hasanuddin.
