Abstract

BACKGROUND: Congenital talipes equinovarus (CTEV) treatment aims to achieve a functional, pain-free, and cosmetically satisfactory foot. The classification of CTEV deformities is important for evaluation before therapy and is useful in evaluating the progress of therapy. The two universally accepted systems for classifying CTEV are the Pirani and Dimeglio classifications. The Ponseti method for managing CTEV has been recognized worldwide as effective and cost-efficient. This study aimed to determine the relationships of the number of serial circular casts required to correct the deformity with the initial Pirani Score before therapy, the initial Dimeglio Score before therapy, and the age at presentation.METHODS: This was a retrospective case-control study of CTEV patients treated with the Ponseti method. The initial Pirani and Dimeglio scores were assessed immediately before treatment. The number of serial castings performed until correction was calculated.RESULTS: The study involved 39 patients. Significant positive correlations existed of the age at presentation, initial Pirani Score, and initial Dimeglio Score with the number of serial castings required to correct CTEV (P<0.05). As these factors increased, the number of serial castings required also increased. This suggests that older age at presentation and higher initial Pirani and Dimeglio scores are associated with more serial castings needed.CONCLUSIONS: The study found significant positive correlations of the patients’ age at presentation, initial Pirani Score, and initial Dimeglio Score with the number of serial castings required to correct CTEV. As these factors increased, the number of serial castings required also increased.
