Comparative study of the effectiveness of transcatheter arterial chemotherapy with and without lipiodol in patients with intermediate-stage hepatocellular carcinoma

Abstract

BACKGROUND: Transarterial chemoembolization (TACE) and transarterial chemoinfusion (TACI) are widely recommended in various clinical guidelines and have become standard treatments for patients with intermediate-stage hepatocellular carcinoma (HCC) who are not candidates for surgical resection. The aim of this study was to compare the effectiveness of transcatheter arterial chemotherapy with and without lipiodol in patients with intermediate-stage HCC.METHODS: An observational analytic study with a cross-sectional design was conducted, using medical records from patients treated between January 2022 and December 2023. Data were analyzed using SPSS software, with a P value of <0.05 considered statistically significant.RESULTS: A total of 72 patients with intermediate-stage HCC were included. The study found a significant association between transcatheter arterial chemotherapy with and without lipiodol and the Child-Pugh Score after the 1st, 2nd, and 3rd treatments. Additionally, there was a correlation between these treatments and changes in tumor size (mRECIST) and serum α-fetoprotein (AFP) levels across all four treatments. Significant differences were noted in tumor size reduction (as measured by 3-phase CT scans) between treatments with and without lipiodol. However, no significant differences were observed in AFP levels or Child-Pugh classification.CONCLUSIONS: TACE with lipiodol proved to be an effective treatment for patients with intermediate-stage HCC, especially in terms of reducing tumor size.
