BACKGROUND: The incidence of post-total thyroidectomy stridor is rare but could be fatal. The endotracheal tube (ETT) leak test and laryngeal air column width difference using ultrasonography (USG) results could be used as methods to predict stridor. This study compared the two as predictors of post-total thyroidectomy stridor incidence.METHODS: This was an observational study with a prospective cohort approach at Wahidin Sudirohusodo General Hospital, Makassar, Indonesia from December 2023 to February 2024. The sample for this study was all patients who underwent total thyroidectomy. All samples underwent postextubation ETT leakage tests and air column width differences examinations.RESULTS: This study was conducted on 22 patients who underwent total thyroidectomy. In this study, 4 (18.2%) patients who underwent total thyroidectomy experienced stridor. The area under curve results for predicting post-total thyroidectomy stridor for the ETT leak test were a cut-off value of 165.50 mm, while for laryngeal air column width differences the cut-off value was 0.87 cm. Sensitivity (88.9% vs. 83.3%), specificity (100% vs. 100%), and accuracy (95.24% vs. 95.00%) were favorable for both, although the ETT leak test was slightly better compared to laryngeal air column width differences, although not significant.CONCLUSIONS: Postextubation ETT leak test and laryngeal air column width difference using USG could be used to predict post-total thyroidectomy stridor.
