Negative correlation between α-Klotho serum level with Left Ventricular Mass Index: a cross sectional study in chronic kidney disease patients in Wahidin Sudirohusodo Hospital, Makassar, Indonesia

BACKGROUND: In chronic kidney disease (CKD), the α-Klotho serum level, a protein that is mainly expressed in the kidneys and has various functions, including as a co-receptor of fibroblast growth factor-23 (FGF-23), which helps increase the phosphate excretion in the kidney and has anti-fibrotic, anti-proliferative, anti-inflammatory, and protective effects on the endothelium, is in deficient state. Low serum α-Klotho level have been linked to cardiac remodeling and left ventricular hypertrophy (LVH). This study aims to understand the relationship between α-Klotho level, left ventricular mass index, and LVH event in CKD patients.METHODS: This cross-sectional study involved 74 CKD subjects with stage 3 (N.=18), stage 4 (N.=19), stage 5 non-dialysis (N.=20) and stage 5 dialysis (N.=17) distribution at Wahidin Sudirohusodo Hospital and Hasanuddin University Hospital Makassar from August 2022 until October 2022. The ELISA (enzyme-linked immunosorbent assay) kit method was used to determine the α-Klotho level. Echocardiography is performed by an experienced cardiologist. Data analysis using SPSS v. 25 with the Mann-Whitney’s Test and Spearman’s Correlation Test. Statistical test results are considered significant if the P value <0.05.RESULTS: There was a negative correlation between serum α-Klotho level and Left Ventricular Mass Index (LVMI) (R=-0.119), and the mean value of serum α-Klotho level was lower in LVH subjects than non-LVH subjects, 522.7 versus 554.5 pg/mL, but this was not statistically significant (P>0.05). Obtained LVH risk factors, such as gender, age, obesity, hypertension, and diabetes mellitus, are independent of the incidence of LVH in CKD.CONCLUSIONS: There is a negative correlation between α-Klotho level and left ventricular mass index in CKD patients.
