Complete surgical excision is one of the definitive treatments of skin cancer. However, this procedure can cause large defects that affect both cosmetic and functional aspects of the patient. We report a 66-year-old man who came to the Dermatology and Venereology Outpatient Clinic complaining of a lump on his preauricular dextra six months prior, initially in the form of a small pruritic pimple-like appearance. Furthermore, the lesion was enlarged and bled easily within the past three months. A dermatological examination of the right preauricular region found an asymmetrical solitary nodule measuring approximately 2 cm in diameter with irregular edges and hyperpigmented crusting. According to the physical and supporting examination, the patient was diagnosed with SCC and controlled excision with a margin of 1 cm was made, leaving a defect measuring approximately 4 cm in diameter. Closure of the defect with U-Plasty, this flap has a single cutaneous pedicle created by making parallel incisions to free up the donor skin and then advanced to fill the defect. U-plasty offers several benefits and can be an excellent method for closing the large defect in the preauricular area, providing great cosmetic and functional outcomes. A follow-up after 23 days showed excellent cosmetic and functional results.
