Correlation between glomerular filtration rate and parathyroid hormone

Abstract

BACKGROUND: This study investigates the impact of chronic kidney disease (CKD) on the balance of minerals in the body, specifically phosphate and calcium levels, fibroblast growth factor 23 (FGF23), parathyroid hormone (PTH), and 1,25-dihydroxyvitamin D 1,25-(OH)2. The study revealed a progressive increase in PTH levels as the glomerular filtration rate (GFR) decreases, commencing at stage 3 with a GFR of less than 60 mL/min/1.73 m2. The aim of this study was to examine the correlation between GFR with PTH levels.METHODS: This study involved conducting an observational cross-sectional analysis of 60 individuals with CKD. The sample consisted of 20 individuals in stage 3, 20 in stage 4, and 20 in stage 5 who were not undergoing hemodialysis. The study was conducted at Wahidin Sudirohusodo Hospital and Unhas Hospital starting from August 2023. PTH levels were quantified using an Enzyme-Linked Immunosorbent Assay (ELISA) kit.RESULTS: The study revealed a correlation between CKD stages and PTH levels, with the highest average PTH level of 158.6 pg/mL observed at stage 5. In stage 3, the PTH levels were 58.1 pg/mL, while in stage 4, they were 51.3 pg/mL. Sample bias may have occurred, thus the potential for elevated PTH levels at stage 4 cannot be dismissed. This was demonstrated in stage 5 non-HD, where the levels of PTH rose to 158.6 pg/mL.CONCLUSIONS: The results of this study demonstrated that there is a significant correlation between GFR and PTH levels.
