Comparison of mean flow velocity, peak systolic velocity and cerebrovascular reactivity between migraine without aura and tension-type headache in interictal phase

Abstract

BACKGROUND: The most common primary headaches are tension-type headaches and migraines, with a global active prevalence of 40% and 10%, respectively. Various studies have examined the role of cerebrovascular factors in migraines, but few have focused on tension-type headaches or directly compared the two. This study aimed to compare the mean flow velocity (MFV), peak systolic velocity (PSV), and cerebrovascular reactivity (CVR) values between migraine without aura and tension-type headache.METHODS: This was an analytical cross-sectional study conducted on 44 subjects with migraine without aura and 44 subjects with tension-type headaches, all during headache-free episodes, who presented to the Neurology Clinic of Wahidin Sudirohusodo Hospital, Makassar, Indonesia, from April to July 2024. All subjects underwent transcranial Doppler examination to assess MFV, PSV, and CVR.RESULTS: The mean MFV in the migraine without aura group was notably greater than in the tension-type headache group (75.30±11.59 cm/s vs. 64.02±13.40 cm/s, P≤0.001). Similarly, the mean PSV in the migraine without aura group was significantly higher compared to the tension-type headache group (115.70±15.33 cm/s vs. 99.36±19.10 cm/s, P≤0.001). Additionally, the average CVR in the migraine without aura group was significantly elevated compared to the tension-type headache group (61.84±13.10 cm/s vs. 54.48±15.36 cm/s, P=0.018).CONCLUSIONS: This study supports the presence of cerebrovascular changes in both conditions, with migraine without aura showing higher MFV, PSV, and CVR values than tension-type headache patients. These findings may serve as important markers to help distinguish between the two types of headaches.
