C-reactive protein as predictors of mortality in elderly pneumonia: a survival analysis

BACKGROUND: Pneumonia has a higher risk in the elderly due to lower immunity. Pneumonia in the elderly is also found to have atypical manifestations. Pneumonia in the elderly is also associated with a higher risk of death. Therefore, it is important to assess the severity and prognosis of elderly pneumonia patients using biomarkers that are simple and easy to examine. This research was conducted to determine the predictive value of C-reactive protein (CRP) levels on the severity and prediction of mortality in elderly pneumonia patients.METHODS: A retrospective cohort study of elderly pneumonia patients hospitalized at RSUP Dr. Wahidin Sudirohusodo from January to December 2023. Patient characteristics and clinical data were collected from inpatient medical records retrospectively. Data analysis used the Kaplan Meier survival curve and Youden index to determine predictors of severity and mortality in elderly pneumonia patients.RESULTS: A total of 654 elderly pneumonia patients were analyzed in this study. The results of the ROC curve analysis show that the AUC value for the CRP variable is 0.538 with the best cut-off value being 15.45. The results showed that there was a significant difference, patients with high levels of CRP had a significantly shorter survival time compared to patients with low CRP values.CONCLUSIONS: CRP can be used as predictors of severity and mortality in elderly pneumonia patients.
